Request a Copy of an Autopsy or Postmortem Report Family
The postmortem report is confidential in the state of Washington per RCW 68.50.105. If you are a family member as defined in RCW 68.50.105 or the personal representative of the decedent (as defined in RCW 11.02.005) and you want a copy of the postmortem report use the Family Request Authorization form.
If you are a third party, such as an insurance company or attorney, you must obtain an authorization from a family member or personal representative. A release for patient medical records (RCW 70.02) may not be substituted to request postmortem records. Use the third party release form for parties not listed in RCW 68.50.105.

Some types of cases may take months for all testing to be returned, the final study, and report completion. Per RCW 36.18.030, duplication fees are $2 for the first page and $1 for each additional page. Please call for the exact duplication fee (cash, check or money order) and enclose the appropriate release form, a copy of photo identification, and the duplication fee when requesting a report by mail.
