Self-Help Forms Packets

The Clerk's Office offers a variety of self-help forms packets for purchase. The packets include step-by-step instructions, along with the proper forms to proceed with your case. The types of packets available are:

  • Domestic/Family Law
  • Establishment of Parentage (Paternity)
  • Domestic Partnership
  • Guardianship
  • Adoption Break Seal
  • Miscellaneous Civil, Criminal and Juvenile

Family Law Cases

If you have questions regarding which family law self-help packet to purchase, you can visit the Family Law Facilitator during their walk-in hours from 8:30 a.m.-11:00 a.m. and 2:30 p.m.-3:30 p.m. Monday through Friday. The Family Law Facilitator cannot fill out forms for you, but can review your forms for completeness prior to going to court. For forms review, please make an appointment by calling the Family Law Facilitators' message line at 425-388-3795. Purchasing Forms Packets

Self-help packets (PDF) are available for purchase online as well as at the Snohomish County Clerk's Office.

Once a packet has been purchased, an email will be sent to the email address you provided with the packet attached in .pdf format. It is the purchaser's responsibility to print out packets that have been purchased online.

If you do not wish to purchase packets online, you can purchase packets at the Snohomish County Clerk's Office, Room M-206 from 8:30 a.m. to 4:30 p.m., Monday through Friday, except County holidays.

Packet prices range from no charge to $35 (see packet list (PDF) for prices). The Clerk's Office accepts cash, money orders, cashier's checks, debit and credit cards. All packet sales are final.

If you do not need an entire packet, some individual forms can also be downloaded from the Superior Court Forms page or the Washington Courts website.
